Office Policies

General

  • Services are by appointment only. Patients must schedule their own appointments.
  • Patients must be at least 18 years old to receive any treatments.
  • No pets are allowed in the mobile medspa.
  • Patients cannot be accompanied by anyone during their appointment unless previously approved by the provider. We are happy to accommodate tours of the mobile medspa before or after the visit if time allows.
  • Children are not allowed in the mobile office at any time.
  • Phone calls, voicemails, and/or text messages will be addressed during listed business hours. You may receive emails or text messages outside of business hours from our patient portal, as these are generated automatically.
  • No personal photos or videos are allowed during treatments without consent of the provider.
  • If you have a medical emergency, please call 911 or go to the nearest Emergency Department.
  • Eos Aesthetics reserves the right to terminate a professional relationship with any patient at any time without cause.

Privacy

Our mobile medspa is HIPAA compliant as is our patient portal, patient communication, and virtual visit platform.


Late Appointments

Since our mobile office comes to you, there are some times where we may encounter unanticipated delays. We will do our best to call to update you on our arrival time or reschedule you for another time that may be more convenient. The rescheduling fee will be waived in these instances. If we are more than 20 minutes late, but you’d like to keep your appointment, you will receive 5% off of your total treatment costs for that day.

If you are late for your appointment, please call and let the office know as soon as you are able. There will be a 20 minute grace period, and after that you may be asked to reschedule your appointment due to the provider not having enough time to appropriately perform your treatment. You will forfeit your booking fee and accrue an additional $25 cancellation fee.


COVID-19 Policies*

  • Our mobile office is keeping up to date with federal and state recommendations regarding masking and indoor gatherings.
  • At this time, we are not requiring masks in our mobile office. This can change based on federal and state recommendations. You are welcome to wear a mask if that is your preference.
  • We clean and sanitize all surfaces between patients in compliance with CDC guidelines.

Booking Info

  • All new patients must have an initial consultation with the aesthetics expert before services are provided.
  • All appointments require a booking fee that is applied towards your treatment costs if you complete your scheduled appointment. The booking fee can be processed over the phone or through your online patient portal, but it must be paid in order for your appointment to be booked.
  • Services can only be booked individually online. Please call the office to discuss booking multiple services in one visit.
  • All new patients must provide their home address when booking. We are a fully mobile office that comes to your home, so we cannot park at businesses or properties without their prior consent.

Travel Charge Policy

  • For appointments requiring a travel time greater than 20 minutes from the Brawley School exit on I-77, an additional travel charge will be applied based on the calculated travel time.
  • Travel time is determined using normal traffic patterns for the chosen appointment time and includes ongoing daily construction delays.
  • Please note that travel times will not be affected by unforeseen delays, including, but not limited to, accidents, stalled vehicles, unexpected weather, law enforcement blockade, unmarked road closures, Rejuvenation Vehicle mechanical issues, or force majeure.
  • Eos Aesthetics does not have a brick and mortar location and is solely a mobile business operated out of the Rejuvenation Vehicle. Appointments cannot take place on the property where the Rejuvenation Vehicle is stored.

Travel Charges

  • Travel time 21 minutes - 30 minutes: $50
  • Travel time 31 minutes - 40 minutes: $100
  • Travel time 41 minutes - 60 minutes: $200

Appointments with a travel time of 60+ minutes are available by request only and must be scheduled via email or phone. Any appointment scheduled online showing an address with a travel time of 60+ minutes will be canceled by Eos Aesthetics, and a refund for the deposit will be issued.

Travel Time Estimation:

  • Travel time will be estimated by Eos Aesthetics once you have input your home address into the Patient Portal or you have provided it by phone or email to our office.
  • We will notify you of any applicable travel charge no later than 24 hours before your appointment.
  • You can then decide to keep the appointment or cancel it (see our Cancellation Policy).

Appointment Location:

  • The address given for the appointment must be privately owned by the patient.
  • Business addresses are only accepted if the property is owned (not leased) by the patient.
  • Public spaces managed by the Parks and Recreation Department, including, but not limited to, parks, playgrounds, and sports complexes, cannot be used for appointments.
  • Again, Eos Aesthetics does not have a brick and mortar location and is solely a mobile business operated out of the Rejuvenation Vehicle. Appointments cannot take place on the property where the Rejuvenation Vehicle is stored.

Waiving the Travel Charge:

The travel charge can be waived on a case-by-case basis, including instances where there are multiple patients being treated at the same location. Please call the office for more information (704) 360-5996.


Payment

  • We accept payment via credit card or debit card. No checks or cash will be accepted.
  • We do not accept or process any medical insurance claims.
  • Though we strive for you to be completely satisfied with your results, all of our services are non-refundable. Our medical grade skincare products are also non-refundable even if they are unused or unopened.
  • Pricing is subject to change without warning.

Parties

We would absolutely love to be invited to inject at an aesthetics party hosted by you! At parties, we will only be using wrinkle relaxers because additional services require a more in depth consultation by our experts.

  • Parties can only have 4-8 people receiving injections for now, but we are happy to show you around our mobile medspa even if you aren’t getting injected.
  • Parties can only have one host.
  • Party hosts will receive a 25% discount on their treatment and/or skincare. Party attendees will receive a 15% discount on their treatment and/or skincare.
  • The party must be scheduled at least 2 weeks in advance. All parties require a credit card on file with Eos Aesthetics, and a deposit is required to secure the date and time.

Cancellation Policy

All services require a booking fee that can be used towards your visit costs. If you cancel your appointment with less than 24 hours notice, you will forfeit your deposit and accrue an additional $25 cancellation fee.

If you re-book another appointment after canceling with less than 24 hours notice, it will be treated as a new appointment, and another booking fee will be collected.


Giftcards

  • You can purchase eGiftcards at your visit or by calling the office.
  • Gift cards are non-refundable and must be used within 1 year of being issued.
  • The money on gift cards can be used towards your visit or be used to purchase our medical grade skin care.
  • Your name has to appear on the gift card to redeem it. Please bring a photo ID.
